I thank Drs. Doorey, Garratt, and Weintraub for correcting my statement and pointing out that the level 1 guidelines for percutaneous coronary intervention following out-of-hospital cardiac arrest currently only apply to patients with ST-segment elevation myocardial infarction. The optimal management of resuscitated patients with out-of-hospital cardiac arrest remains a challenge. Despite encouraging and relatively consistent reports from several case series and propensity-matched registry data (particularly for patients whose initial electrocardiogram is consistent with ST-segment elevation myocardial infarction), data from large, randomized clinical trials do not yet exist.
